Notification of Change Form

The Public Interest Commissioner maintains a list of Chief and Designated Officers for public entities identified under the Public Interest Disclosure (Whistleblower Protection) Act. The list is used to facilitate communications between our office and the various public entities but more importantly assists our investigators in being able to direct potential clients to the appropriate persons within each agency, when appropriate.

We request public entities advise our office as to the names and contact information of Chief and Designated Officers.  We also request that as these officers change you take the time to update us, so we can ensure the most current information is available.
